Job Description

Provide home-based acute, post-acute, and transitional care to medically complex and recently discharged adult and geriatric patients across the San Fernando Valley and Antelope Valley. Work autonomously in the field as part of an interdisciplinary team focused on improving outcomes, preventing readmissions, and supporting safe recovery at home. Primary responsibilities Perform comprehensive assessments, diagnosis, treatment, and ongoing care management for adult/geriatric patients in home settings. Manage acute and post-acute needs, medication reconciliation, wound care, and chronic disease management. Coordinate care with physicians, nurses, therapists, social workers, and community resources. Document visits and communicate clinical status and plan of care to the care team. Participate in quality improvement and value-based care initiatives to reduce hospital utilization. Professional development and career advancement support. Minimum qualifications Master of Science in Nursing. Adult/Geriatric Nurse Practitioner certification. Active, unrestricted California RN and NP licenses. At least 1 year of NP clinical experience. Strong assessment, diagnostic, and care-management skills. Valid driver's license and reliable transportation. Current TB test or willingness to obtain prior to employment.
