Job Description

Where You'll Work Dignity Health's Pacific Central Coast Health Centers (PHC) is a non-profit Community clinic organization comprised of more than 40 health centers located primarily from Ventura to Templeton, spanning the coast. Our experienced physicians and advanced practice providers offer a wide range of primary and specialty care services. PHC is a part of Dignity Health Central Coast's award-winning network of outstanding hospitals, imaging centers, laboratories, and post-acute services. One Community. One Mission. One California Job Summary and Responsibilities As our Care Team Access APP, you will contribute to primary health care for adult and pediatric patients, utilizing your advanced skills to manage various medical conditions. Every day, you will conduct health assessments, perform examinations, and provide support services to physicians in the clinic setting. To be successful in this role, you will be a credentialed Advanced Practice Provider with strong clinical abilities and a commitment to patient care within a medical setting. Take patients' medical history and perform physical examinations for any presenting problem(s) Order specific laboratory studies and x-rays, and other studies, as appropriate Requests blood draws as required, orders collection and other tests as indicated Counsel patients and their families on health promotion, diagnosis, prognosis and management possibilities May make necessary comprehensive referrals as required Diagnosis and prescribe treatment as needed Job Requirements Required Master of Science Registered Nurse Practitioner in the state of California
