Competitive per diem compensation, based on experience Optima Health is seeking an experienced and compassionate Nurse Practitioner (NP) to provide mobile Primary Care throughout the High Desert region . This per diem position delivers high-quality medical care directly to older adults residing in Senior Living Communities . The Mobile Nurse Practitioner works autonomously in the field while collaborating closely with Optima Health physicians, clinical teams, community staff, residents, and families. The ideal candidate is comfortable managing medically complex older adults, making sound clinical decisions, and providing consistent, relationship-based Primary Care in a mobile setting. Key Responsibilities Provide comprehensive Primary Care to patients within Senior Living Communities throughout the High Desert region Perform patient evaluations, physical examinations, and routine follow-up visits Diagnose, treat, and manage acute and chronic medical conditions Develop and update individualized treatment and care plans Review medications and perform medication reconciliation Order and interpret laboratory testing, imaging, and other diagnostic studies as clinically appropriate Coordinate care with physicians, specialists, community staff, pharmacies, home health, hospice, and other healthcare partners Identify changes in patient condition and intervene appropriately Educate patients, families, and community staff regarding diagnoses, medications, treatment plans, and preventative care Maintain timely and accurate clinical documentation in the electronic medical record Follow all applicable clinical, regulatory, and organizational standards Communicate effectively with Optima Health's internal clinical and operational teams to support continuity of care Qualifications Active and unrestricted California Nurse Practitioner license Current furnishing number and prescribing authority as required Current DEA registration, if applicable Board certification as a Nurse Practitioner Current BLS certification Strong clinical assessment and decision-making skills Ability to practice autonomously in a mobile environment Reliable transportation and willingness to travel throughout the assigned High Desert territory Strong communication, documentation, and organizational skills Preferred Experience 1-2+ years of Nurse Practitioner experience preferred Primary Care , internal medicine, geriatrics, or family medicine experience Experience treating older adults and medically complex patients Experience providing care in Senior Living Communities , assisted living, memory care, home-based Primary Care , or other mobile healthcare settings Schedule This is a per diem position , generally supporting 1-3 clinical days per week based on mutual availability and community needs . Scheduling is coordinated in advance, with providers traveling between assigned Senior Living Communities throughout the High Desert region.
