This role offers the opportunity to provide comprehensive, patient-centered care in a collaborative outpatient setting serving diverse and underserved populations. Position Overview The Advanced Practice Clinician will deliver high-quality medical services in a community health clinic environment, providing family practice care to patients of all ages. This role works closely with physicians, medical leadership, and interdisciplinary care teams to manage acute and chronic conditions, coordinate referrals, and support continuity of care. Key Responsibilities Provide comprehensive primary care services including evaluation, diagnosis, and treatment of acute and chronic medical conditions Perform physical examinations, review medical histories, interpret diagnostic and laboratory results, and develop appropriate treatment plans Prescribe medications and therapeutic interventions within scope of licensure and established clinical protocols Monitor patient response to treatment and initiate appropriate follow-up, referrals, or emergency procedures when necessary Administer medications and therapeutic agents as part of treatment and preventive care plans Educate and counsel patients on health conditions, preventive care, and treatment adherence Maintain accurate and timely medical documentation in accordance with regulatory and organizational standards Collaborate with the Medical Director and care team to ensure high-quality, coordinated patient care Perform additional clinical duties as assigned Qualifications Nurse Practitioner or Physician Assistant with active California licensure Nurse Practitioners must hold current RN and NP licenses with board certification Physician Assistants must hold a California PA license, PA certificate, and NCCPA certification Current BLS and DEA certification required One to two years of clinical experience preferred, particularly in community health, underserved, or low-income patient populations Bilingual proficiency in English and Spanish, Russian, Farsi, or Pashto/Pushto is preferred Strong clinical judgment, communication skills, and commitment to patient-centered care Additional Requirements Valid California driver's license and proof of personal auto insurance Ability to meet immunization, tuberculosis, and vaccination requirements prior to start date Successful completion of background check and drug screening Ability to perform essential physical functions of a clinical role, including standing, walking, and occasional lifting Compensation Salary is dependent upon experience and qualifications Benefits Retirement plan with employer matching and immediate vesting Medical, dental, and vision insurance options Health reimbursement arrangement and flexible spending accounts Paid time off and paid holidays Malpractice insurance with tail coverage Continuing education and tuition reimbursement programs Employee assistance and wellness resources Eligibility for state loan repayment programs Interested candidates are encouraged to apply to learn more about this opportunity and how they can make a meaningful impact in their community through patient-focused care.
