Job Summary We are seeking a dedicated and energetic Midlevel Provider (Nurse Practitioner or Physician Assistant) to join our team at a leading Skilled Nursing Facility. EMRs:

PCC & Gehrimed Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to determine health status. Develop and implement individualized care plans in collaboration with physicians, nurses, and other healthcare professionals. Prescribe medications and treatments following state regulations Manage acute and chronic conditions common in skilled nursing populations, including geriatrics, dementia care, and complex comorbidities. Document all patient interactions accurately within Electronic Health Record (EHR) while ensuring compliance with HIPAA regulations. Participate in interdisciplinary team meetings to coordinate discharge planning, medication management, and ongoing patient education. Qualifications Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) with appropriate state certification. Proven experience working in skilled nursing facilities or long-term care settings; experience in nursing homes preferred. Strong understanding of geriatrics, dementia care, hospice & palliative medicine, and post-acute care protocols. Ability to perform clinical evaluations involving physical examinations and diagnostic assessments like sonography or laboratory specimen collection. Excellent communication skills to collaborate effectively with healthcare professionals and educate patients' families about treatment plans.
