OnPoint Medical Group seeks an Advanced Practice Practitioner to deliver compassionate, patient-centered care in our Colorado primary care and specialty clinics. In this collaborative, physician-led group, you will evaluate and manage acute and chronic conditions, perform routine procedures, coordinate with behavioral health and ancillary teams, and emphasize prevention and wellness. You will build long-term relationships with patients, document in the EHR, and contribute to quality improvement while impacting community health across the Denver metro area.
