Job Description

OnPoint Medical Group seeks an Advanced Practice Practitioner to deliver compassionate, patient-centered care in our Colorado primary care and specialty clinics. In this collaborative, physician-led group, you will evaluate and manage acute and chronic conditions, perform routine procedures, coordinate with behavioral health and ancillary teams, and emphasize prevention and wellness. You will build long-term relationships with patients, document in the EHR, and contribute to quality improvement while impacting community health across the Denver metro area. Responsibilities Provide comprehensive primary and specialty care as an Advanced Practice Practitioner in a coordinated, patient-centered model Assess, diagnose, and manage acute and chronic conditions for diverse patient populations Perform routine procedures, interpret diagnostic tests, and manage medications per evidence-based guidelines Collaborate closely with physicians, behavioral health, and ancillary teams to optimize outcomes Educate patients and families, promoting prevention, wellness, and long-term relationships Document care accurately in the EHR and participate in quality and process improvement initiatives Required Skills Advanced clinical assessment Diagnosis and treatment planning Chronic disease management Primary care procedures Electronic Health Record (EHR) use Patient and family education Care coordination Evidence-based practice Prescribing/medication management Interdisciplinary collaboration
