Job Description

Boone Heart & Wellness is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) with experience in hormone replacement/optimization to join our growing preventive and lifestyle medicine practice in Fort Collins. This role focuses on patient-centered, root-cause medicine with extended visit times and a collaborative care model.
Responsibilities:
Provide patient care 3-4 days/week, including: o 90-minute new patient visits o Annual wellness visits o 30-60 minute follow-ups o Telemedicine appointments Conduct patient assessments: o Medical history, physical exams o Lab and imaging interpretation o Cardiovascular, metabolic, and hormone evaluations Develop and manage individualized treatment plans using preventive and functional medicine approaches Treat chronic conditions including: o Cardiovascular disease o Metabolic dysfunction o Obesity Evaluate and manage : o Hormone imbalances (menopause, andropause, thyroid) o Functional gastrointestinal conditions Educate patients on: o

  • o• Lifestyle and stress management • o• Hormone therapy • Order and interpret labs and diagnostics Collaborate with physicians and care team members Maintain accurate, compliant medical records ________________________________________

$57.69 - $72.11 per hour
