Job Description
Regional Hospice is looking for per diem Advance Practice Nurses with hospice and palliative care experience for our inpatient Center for Comfort Care & Healing (12 bed hospice residence in Danbury) and also with our Home Hospice and Palliative Care programs in the community. Conduct in person "face-to-face" patient assessments and collaborate with interdisciplinary team regarding patients' plans of care. Our Home Hospice and Palliative Care programs serve both adult and pediatric patients throughout Fairfield, Litchfield and New Haven counties. Position also includes rotating weekend availability for our inpatient Center.
