Healthcare Professionals accepted) Psychiatry/Mental Health-NP - A regional community health center covering a thirty seven town rural region in eastern Connecticut providing provides a full range of preventative and primary care medical services to all ages including health maintenance of chronic diseases, 24 hour on call, hospitalization coverage and patient case management, as well as complete behavioral health and dental services is seeking a psychiatric nurse practitioner to join their multi-disciplinary healthcare team. The psychiatric Nurse Practitioner provides coordinated psychiatric services to patients utilizing the clinical services of the Health Center. Clinical services provided shall include routine and crisis adult and pediatric care as assigned. The scope of practice includes evaluation, diagnosis, medication assessments and adjustment, patient education, and treatment planning. The APRN will be knowledgeable about agency and clinical policies, keep patient information confidential, and shall assume responsibility for the development and realization of an individual ongoing care plan for all patients.
