Nurse Practitioner or Physician Assistant, Swank Urgent Care and Inboxologist

Job Details ChristianaCare is currently seeking seeking a dedicated and compassionate Memory Care Specialist APC to join our team. This role can be filled by a candidate with experience in Geriatrics/FNP, Psychiatry, or Neurology with a passion for providing exceptional care to patients with memory disorders, including Alzheimer's disease and other forms of dementia. The ideal candidate will have a strong background in diagnosing, treating, and managing cognitive impairments and will work collaboratively with a multidisciplinary team to enhance the quality of life for our patients. Candidates with specialty/fellowship training are encouraged to apply. •This position will support the Swank Urgent Care and Inboxologist patients.

Work Schedule:

Monday - Friday, day shift, no weekends, holidays, or on-call required

Key Responsibilities:

Conduct comprehensive assessments and evaluations of patients with memory disorders. Develop and implement individualized care plans tailored to the needs of each patient. Provide ongoing medical management and follow-up care for patients with cognitive impairments. Collaborate with other healthcare professionals, including nurses, social workers, and therapists, to ensure a holistic approach to patient care. Educate patients and their families about memory disorders, treatment options, and strategies for managing symptoms. Participate in clinical research and stay updated on the latest advancements in memory care. Maintain accurate and detailed medical records in compliance with healthcare regulations. Provide leadership and guidance to the memory care team.

Qualifications:

Masters degree (NP or PA) from an accredited institution. Board certification in Geriatrics, Psychiatry, or Neurology. Valid license to practice in DE. Strong interpersonal and communication skills. Compassionate and patient-centered approach to care. Ability to work effectively in a multidisciplinary team environment. Commitment to ongoing professional development and education.
