Position Summary The Wound Care Nurse Practitioner will provide comprehensive evaluation, treatment, and ongoing management of patients with chronic and complex wounds in home and community settings. This provider will practice autonomously while collaborating closely with our clinical and operations teams to deliver outstanding outcomes. Responsibilities Perform comprehensive wound assessments and develop individualized treatment plans Diagnose and manage chronic and complex wounds Perform sharp and conservative debridement as clinically appropriate and within scope of practice Apply advanced wound care products and skin substitutes as indicated Order and interpret diagnostic studies and laboratory testing Coordinate care with primary care providers, specialists, home health agencies, and facility staff Complete accurate, timely electronic documentation Educate patients, families, and caregivers on wound prevention and treatment Participate in quality improvement initiatives and clinical collaboration QualificationsRequired Current Florida APRN License National Nurse Practitioner Certification DEA Registration Active BLS Certification Valid Florida Driver's License and reliable transportation Excellent clinical assessment and communication skills Preferred Wound Care Certification (WCC, CWCN, CWON, CWS, or equivalent) Experience in wound care, vascular medicine, home health, hospice, geriatrics, or primary care Knowledge of advanced wound therapies and skin substitutes Experience working independently in community settings What Makes Oasis Different We intentionally keep our focus on quality over volume.
