09/07/2026 Job Description Job Description Geriatric Nurse Practitioner (ARNP) - Mobile Facility-Based Care South Florida (Broward / Palm Beach County) About Us Florida Internal Medicine Associates is a growing, patient-centered practice focused on delivering high-quality, compassionate care to geriatric populations in Assisted Living Facilities (ALFs), Independent Living Facilities (ILFs), and Skilled Nursing Facilities (SNFs). We emphasize a holistic, "inside-out" approach to wellness, chronic disease management, and preventive care. Position Overview We are seeking a motivated and experienced Geriatric Nurse Practitioner (ARNP) to provide on-site medical care to patients in long-term care and residential facilities. This is a mobile position ideal for a provider who enjoys autonomy, building relationships, and making a meaningful impact in the elderly population. Responsibilities Provide primary and preventative care to geriatric patients in ALF, ILF, and SNF settings Perform comprehensive assessments, routine follow-ups, and acute visits Manage chronic conditions (HTN, diabetes, dementia, etc.) Coordinate care with facility staff, specialists, and families Complete documentation in EMR in a timely manner rotating weekend call Participate in care planning and interdisciplinary team meetings Ensure compliance with all regulatory and clinical guidelines Qualifications Active Florida ARNP license Board Certified (ANCC/AANP) Medicare credentialed a plus Experience in geriatrics, internal medicine, or long-term care preferred Strong clinical assessment and diagnostic skills Ability to work independently and manage a flexible schedule Reliable transportation (travel between facilities required) Compensation & Structure 1099 Independent Contractor (flexible schedule) Established patient base + support with scheduling and billing Opportunity for growth within a rapidly expanding practice Why Join Us Flexible work-life balance Autonomy with strong administrative support Meaningful patient relationships Growing practice with advancement opportunities How to Apply Please submit your resume and availability.
