Opportunity:

Monday - Friday, 8am-5pm Rotating and flexible on-call schedule. Average daily volume is 12-15 patients per day We use eClinical Works (eCW), along with partnering programs to ensure ease of use for our patients and practices. We focus on continuity of care, health maintenance, and disease prevention, working alongside patients and their families to prevent, diagnose, treat and manage a Medicare Advantage population with multiple comorbidities.

Integrated team-based care:

Working as a multidisciplinary team to improve patient outcomes by meeting their needs and preferences. Prioritizing preventative care and proactive management of chronic conditions to improve patient outcomes, decrease patient's costs, and increase revenue for the practice. Providers must be comfortable managing a primarily Medicare-aged patient population.
