Family Nurse Practitioner (FNP-BC) / Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) Infinity Health Community Clinic (IHCC)Full-Time PositionMonday - Friday | 9:00 AM - 5:30 PM About Infinity Health Community Clinic Infinity Health Community Clinic (IHCC) is a growing nonprofit healthcare organization serving communities across South Florida. We provide comprehensive primary care, mental health services, HIV prevention and treatment, Hepatitis C care, STI testing and treatment, and community outreach programs. Our mission is to expand access to high-quality, compassionate healthcare for underserved populations. Position Summary IHCC is seeking a compassionate, motivated Dual-Certified Family Nurse Practitioner (FNP-BC) and Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) to deliver integrated primary care and behavioral health services. This role combines clinic-based care with community outreach, offering the opportunity to make a meaningful impact on diverse patient populations. The ideal candidate is confident working independently, committed to patient-centered care, and passionate about improving community health outcomes. Essential Duties and Responsibilities Deliver comprehensive primary care services for patients across the lifespan. Conduct psychiatric assessments and provide medication management. Diagnose and treat acute and chronic medical conditions. Develop individualized treatment plans addressing both physical and behavioral health needs. Prescribe medications in compliance with state and federal regulations. Perform annual wellness exams, preventive care, and health screenings. Manage patients receiving HIV treatment, PrEP, PEP, Hepatitis C treatment, and STI-related services. Collaborate with therapists, case managers, medical assistants, and community partners to coordinate care. Maintain accurate, timely documentation in the electronic medical record (EMR). Participate in community outreach initiatives, including mobile clinics, shelters, recovery centers, and partner organizations. Educate patients on disease prevention, medication adherence, and healthy lifestyle practices. Support quality improvement efforts and regulatory compliance initiatives. Qualifications Current Florida APRN license in good standing. Board Certification as a Family Nurse Practitioner (FNP-BC). Board Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC). Active DEA registration. Current BLS certification. Two or more years of clinical experience preferred. Experience with HIV care, PrEP, PEP, Hepatitis C, substance use disorders, and mental health services preferred. Strong communication and interpersonal skills. Ability to work independently and collaboratively within a multidisciplinary team. Reliable transportation and willingness to travel throughout Miami-Dade, Broward, and Palm Beach Counties. Work Environment Combination of office-based clinical care and community field work. Full-time schedule, Monday through Friday, 9:00 AM to 5:30 PM.
