Position Summary IHCC is seeking a compassionate, motivated Dual-Certified Family Nurse Practitioner (FNP-BC) and Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) to deliver integrated primary care and behavioral health services. This role combines clinic-based care with community outreach, offering the opportunity to make a meaningful impact on diverse patient populations. Essential Duties and Responsibilities Deliver comprehensive primary care services for patients across the lifespan. Conduct psychiatric assessments and provide medication management. Diagnose and treat acute and chronic medical conditions. Develop individualized treatment plans addressing both physical and behavioral health needs. Prescribe medications in compliance with state and federal regulations. Perform annual wellness exams, preventive care, and health screenings. Manage patients receiving HIV treatment, PrEP, PEP, Hepatitis C treatment, and STI-related services. Collaborate with therapists, case managers, medical assistants, and community partners to coordinate care. Maintain accurate, timely documentation in the electronic medical record (EMR). Participate in community outreach initiatives, including mobile clinics, shelters, recovery centers, and partner organizations. Educate patients on disease prevention, medication adherence, and healthy lifestyle practices. Support quality improvement efforts and regulatory compliance initiatives. Qualifications Current Florida APRN license in good standing. Board Certification as a Family Nurse Practitioner (FNP-BC). Board Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC). Active DEA registration. Current BLS certification. Two or more years of clinical experience preferred. Experience with HIV care, PrEP, PEP, Hepatitis C, substance use disorders, and mental health services preferred. Strong communication and interpersonal skills. Ability to work independently and collaboratively within a multidisciplinary team. Reliable transportation and willingness to travel throughout Miami-Dade, Broward, and Palm Beach Counties. Work Environment Combination of office-based clinical care and community field work. Full-time schedule, Monday through Friday, 9:00 AM to 5:30 PM. Assist with telehealth consultations and triage services to deliver accessible healthcare remotely when appropriate. Qualifications Valid state licensure as a Family Nurse Practitioner with board certification from an accredited certifying body. Proven experience in diverse clinical environments such as hospital medicine, emergency medicine, urgent care, or nursing homes. Strong knowledge of medical terminology, physiology, anatomy, ICD coding (including

), CPT coding for procedures and billing purposes. Experience with EMR/EHR systems such as Epic, Athenahealth, Cerner or eClinicalWorks is highly preferred. Demonstrated skills in performing procedures like dermal fillers, botulinum toxin treatment (for esthetic purposes), spinal taps, sonography, gastrostomy tube feeding or ventilator management. Critical care experience including ICU or Level I/II trauma center exposure is advantageous. Ability to work with diverse populations including pediatrics (including neonatal ventilator management), geriatrics (memory care/dementia care), hospice & palliative medicine patients or individuals with developmental disabilities. Familiarity with infection control protocols, infection prevention strategies and aseptic techniques essential for safe practice. Excellent patient assessment skills combined with the ability to perform diagnostic evaluations using laboratory experience or medical imaging when necessary. Strong communication skills for effective collaboration within multidisciplinary teams and providing exceptional patient care across outpatient or inpatient settings.
