As an APRN, you will play a vital role in delivering comprehensive healthcare services, performing advanced assessments, and implementing treatment plans that improve patient outcomes. General Summary:
This position is responsible for providing direct primary health care (Acute Care and Chronic Care) to Free Medical Clinic patients in the Daytime Chronic Clinic under the supervision of an assigned physician.
Essential Duties and Responsibilities:
Direct Patient Care Provide direct care to Pioneer Medical Foundation Free Medical Clinic patients, focusing on health promotion and treatment of acute and chronic illness needs. Identify, develop, and implement patient education programs. Orient the patients to the Clinic system and available services. Act as an advocate in directing patients to available Clinic and community resources. Order appropriate cost-effective diagnostic tests or medications. Interpret findings to form diagnoses. Negotiate appropriate treatment plans with patients. Prescribe medications as allowed by practice agreement with supervising physician. Educate patients regarding the condition, treatment, and possible outcomes. Consult with the Clinical Services Coordinator, Executive Director, and Medical provide appropriate and cost-effective medications to patients. Perform complete and focused physical exams as indicated. Provide health care services for identified prevention and health promotion needs. Consult with the supervising physician as needed. Collaborate with other members of the staff and with volunteers. Make appropriate referrals. Evaluate and modify treatment as indicated by patient outcomes. Administrative Maintain accurate medical records using Pioneer Medical Foundation Clinic EMR (Athena) and proper forms in a timely manner. Supervise and train clinical staff and volunteers on data entry into the electronic health records system and provide oversight to ensure that the original data is entered correctly. Track patient data that will demonstrate the measurable success of Clinic services. Generate and analyze reports to document outcome measures from the electronic records. Must be proficient in developing & analyzing outcome measures. Assist the Foundation Clinic Manager in supervising nurse practitioner student volunteers. Consistently use supplies and equipment in a cost-effective manner. Recommend needed supplies and equipment for the delivery of care. Quality Assurance Participate in quality improvement activities as directed by the Medical Director. Provide education to clinical staff as needed. Communicate effectively with patients and their families in a respectful manner. Review and discuss selected charts quarterly with the supervising physician. Maintain an acceptable caseload to meet productivity needs. Ensure that care provided is consistent with community standards of care within the limitations of the program. Follow established safety precautions in the performance of duties to ensure a safe environment. Support policies and philosophy in delivering health care as set forth by the Free Medical Clinic Board of Directors. Other duties as assigned. Education/Vocational/Previous Experience Recommendations:
Requires successful completion of a Master's level family or Adult Nurse Practitioner Program and a Prescriptive Authority granted by the Florida State Board. Certified and Licensed as a Nurse Practitioner in the State of Florida. Licensed to prescribe medication in the State of Florida. A minimum of 1 year of experience in a primary care setting is preferred. Must have strong Microsoft Office computer skills and be proficient in EMR. Public health experience or experience working with the underserved and or uninsured population preferred. Bilingual English/Spanish proficiency is desirable.
Working Conditions:
Clinic on-site work schedule. May require occasional travel between locations. Regular physical activity includes walking, standing, bending, and reaching. Occasional need to lift or move equipment weighing up to 50 pounds. Ability to manage multiple priorities and maintain composure under stress.
Pay:
$100,000.00 - $115,000.00 per year
