Responsibilities The goal of Northside Hospital's (NSH) Palliative Medicine program is to focus on the physical, emotional, and spiritual needs of patients facing complex or life limiting illness at any point during their disease process. The goal of this program is to reduce suffering and improve quality of life for these patients and their families in collaboration with other members of the health care team. As a member of the Palliative Interdisciplinary team, this position will play a pivotal role in providing palliative medicine consultation for seriously ill patients and their families. Duties include comprehensive medical and psychosocial evaluation of patients and families, skilled and meticulous symptom management, and involvement and participation in discharge planning and continuity of care efforts according to NSH standards of care. Educational efforts in palliative medicine are key elements of this work. Promotes excellence in care and customer service to all patients served. Serves as a role model of excellence in nursing knowledge and clinical practice. Qualifications
REQUIRED
Master's Degree in Nursing from an Accredited College or University and certified as a Nurse Practitioner in area of clinical focus. Minimum three (3) years' experience in nursing; experience in oncology, hospice, geriatrics or palliative medicine. Certification in palliative care, Certified Hospice and Palliative Nurse (CHPN) or Advanced Certified Hospice and Palliative Nurse (ACPHN), within (24) twenty four months of hire or transfer. BLS Certification every two (2) years.
PREFERRED ACLS
Certification every two (2) years.
