CVS Health is seeking an Advanced Practice Provider (NP/PA) to deliver compassionate, high-quality care in a patient-centered clinical setting. In this role, you'll provide primary and preventive services, manage acute and chronic conditions, and coordinate care across CVS Health's integrated network. Responsibilities include comprehensive assessments, diagnosis, treatment planning, ordering and interpreting diagnostics, and prescribing medications. You'll document in the EHR, support quality and access goals, and collaborate with diverse teams in an inclusive, purpose-driven culture focused on better health outcomes. Responsibilities Deliver comprehensive primary and preventive care, including assessments, diagnosis, and treatment for diverse patient populations. Manage acute and chronic conditions following evidence-based guidelines and CVS Health clinical protocols. Order, interpret, and act on diagnostic tests, labs, and imaging to inform care plans. Prescribe and manage medications, ensuring safety, adherence, and cost-effective options. Educate patients and families on conditions, treatment plans, and healthy lifestyle practices. Collaborate with pharmacists, physicians, nurses, and care teams to coordinate integrated care. Document care thoroughly and accurately in the Electronic Health Record (EHR). Support quality, safety, and performance metrics, including patient satisfaction and clinical outcomes. Participate in continuous learning, training, and quality-improvement initiatives.
