Ascend Medical is seeking a compassionate and experienced Nurse Practitioner or Physician Assistant to provide relationship-centered care at premier Senior Care communities in Sugar Hill and Grayson, GA. This part-time role is ideal for a provider who is passionate about geriatrics and values quality time with patients over quantity. You'll be supported by a dedicated clinical care team, innovative technology, and full operational support so you can focus on what matters most-delivering exceptional care and building trusted relationships. Build trust with patients, families, and facility staff through clear communication and consistent presence. Manage chronic conditions, address acute concerns, and tailor care plans to the unique goals of each patient. Collaborate with the facility's care team to ensure seamless, coordinated support. Requirements Qualifications Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Georgia (unrestricted, non-probationary). Master's degree from an accredited program. Minimum of 3 years of clinical experience, with strong preference for geriatric, hospice, or home health background. Active DEA license (or eligibility to obtain). Availability to work on-site in Sugar Hill and Grayson; occasional visits to additional locations may be required. Salary Description from $70 per hour
