Job Summary We are seeking a dynamic and compassionate Primary Care Nurse Practitioner to join our healthcare team. In this vital role, you will provide comprehensive, patient-centered primary care services across diverse populations, including pediatrics, geriatrics, and adults. Your expertise will support health promotion, disease prevention, and management of acute and chronic conditions. You will collaborate closely with physicians, specialists, and other healthcare professionals to deliver exceptional care in a fast-paced environment. This position offers an exciting opportunity to make a meaningful difference in patients' lives through proactive health interventions and personalized treatment plans. Responsibilities Conduct thorough patient assessments, including physical examinations and vital signs, to develop accurate diagnoses. Prescribe medications and treatment regimens in accordance with state regulations and collaborative practice agreements. Manage a broad scope of clinical procedures such as suturing, IV infusion, catheterization, and wound care using sterile techniques. Utilize electronic health record (EHR) systems like Epic, Cerner, or eClinicalWorks for documentation, order entry, and medical records management. Provide health coaching and education tailored to individual patient needs across age groups, including pediatrics and geriatrics. Perform diagnostic evaluations using laboratory testing, sonography, or medical imaging to inform clinical decisions. Collaborate effectively with healthcare professionals across specialties such as gastroenterology, infectious disease care, behavioral health, and critical care units to optimize patient outcomes. Experience Proven experience working as a Nurse Practitioner in primary care settings with a strong background in medical office procedures. Demonstrated competence in managing acute pain, chronic conditions, and complex cases within hospital or outpatient environments. Familiarity with EMR/EHR systems such as Epic or Athenahealth for efficient documentation and patient data management. Experience working with diverse populations including children, seniors in assisted living or nursing home settings, and individuals with disabilities or developmental disabilities. Knowledge of healthcare diagnosis coding systems like
ICD-10/ICD-9/CPT
coding for accurate billing and documentation. Prior involvement in telehealth services or remote patient monitoring initiatives is preferred. Critical care experience from ICU or Level I/II trauma centers enhances your ability to handle urgent situations effectively.
Pay:
$120,000.00 - $125,000.00 per year
