Job Description

Meza Post Acute and Long Term Care (MezaCare) is looking for a Nurse Practitioner or Physician Assistant to join our expanding and growing team of 23 providers! MezaCare exclusively serves the geriatric population residing in independent retirement centers, assisted living facilities and skilled nursing facilities In North Idaho and the Spokane Valley area. In this role, you would be delivering primary care to Skilled Nursing and/or Assisted Living residents right where they live. The position is 4 days per week with an average number of visits per day of 16. You would be responsible for managing health problems and coordinating health care for our geriatric population in accordance with State and Federal rules and regulations and nursing and medical practice standards of care. The provider functions independently but with the support of the rest of the team to perform age-appropriate history and physical for geriatric patients; orders and interprets diagnostic and therapeutic tests relative to patient's age-specific needs; Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities Assists with the provision of surgical care in accordance with facility, state, and federal regulations. Monitors the effectiveness of interventions. Facilitates the patient's transition within and between health care settings, e.g. admitting, transferring, and discharging patients. Collaborates with multidisciplinary team members by making appropriate referrals. Facilitates staff, patient and family decision making by providing educational tools.
