Help Us Change What Primary Care Looks Like for Older Adults Every day, medically complex older adults in skilled nursing facilities and assisted living communities go without the consistent, attentive primary care they deserve. Too often their care is fragmented and reactive - changes in condition caught late, avoidable ER visits, and a revolving door back to the hospital. We're building something different. At Community Physicians, our primary care program brings structured, proactive, relationship-based medicine directly to where our patients live. We're looking for a Nurse Practitioner or Physician Assistant to be the primary care backbone for residents across our SNF and assisted living communities in Barrington and Hoffman Estates, not just rounding on patients, but owning a panel alongside a physician and measurably changing outcomes. What You'll Do Working hand-in-hand with our physician, you will deliver longitudinal primary care across a two top-tier CCRC's (Continuing Care Retirement Communities) in Barrington and Hoffman Estates, which include of skilled nursing, assisted living, independent living, and memory. Serve as the primary care provider for a consistent panel of SNF and AL residents Conduct structured rounding following standardized visit cadence and quality protocols developed by our physician team Perform admission evaluations, follow-up visits, chronic disease management, and acute visits Catch changes in condition early and intervene to prevent avoidable ER visits and hospital readmissions Partner with interdisciplinary teams, facility staff, consultants, and families to coordinate care and support smooth transitions Your patient population: older adults with heart failure, COPD, diabetes, dementia, frailty syndromes, polypharmacy, and the full range of chronic conditions of aging, in both post-acute recovery, assisted living, and long-term residential care.
