Job Description

Nurse Practitioner•Chicago, IL Job type:
Full Time Shifts:
Mon•Fri (9am•5pm)

Job Summary & Responsibilities:
Provide comprehensive care in a primary care setting, addressing preventive, acute, and chronic needs. Perform age-appropriate history, physical exams, diagnostics, and treatments. Independently manage common and complex conditions; escalate critical cases appropriately. Collaborate with supervising physicians as needed. Prescribe and implement both pharmacologic and non-pharmacologic interventions. Evaluate treatment effectiveness and adjust as necessary. Communicate effectively with patients, families, and the care team. Perform approved medical procedures based on patient needs. Educate patients and families at an appropriate comprehension level; support quality improvement initiatives.
Education, Licensure, and Experience:
Minimum 2 years of clinical experience, preferably in an FQHC or community health center Experience in both adult and pediatric care Master's degree in Nursing or Physician Assistant Studies from an accredited program Current state license as a Nurse Practitioner or Physician Assistant Family Medicine certification preferred Valid CPR provider card Current DEA and Controlled Substance License (if applicable) Strong communication skills; ability to function in both leadership and team roles Basic computer skills and familiarity with electronic health records (EHR) Ability to respond effectively to medical emergencies For more information please reach out to
