Job Description
Position Summary:
The Nurse Practitioner works within the Harm Reduction framework to provide low-barrier, trauma-informed medical care to individuals experiencing homelessness and those disconnected from traditional healthcare systems. This role delivers mobile medical services across TNM's outreach platforms, including Street Medicine, Health Outreach Bus, CTA outreach, shelter-based services, and youth programming, while bridging individuals to ongoing primary care, specialty care, and other community-based services.General Purpose:
The purpose of this role is to deliver comprehensive, harm-reduction-oriented medical care in nontraditional settings; reduce barriers to care for unsheltered and marginalized populations; support engagement in substance use treatment, mental health care, and chronic disease management; strengthen referral pathways and partnerships; ensure compliance with regulatory and funding requirements; and contribute to ongoing program development and quality improvement.Role Qualifications:
Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program Licensed Advanced Practice Nurse in Illinois (Family preferred; Adult-Gerontology or other relevant population focus considered) DEA license or eligibility to obtain Minimum 2+ years of clinical experience; primary care or community health preferred Experience working with individuals experiencing homelessness, substance use disorders, serious mental illness, or complex trauma strongly preferred Ability to work evenings and weekends Valid driver's license and good driving recordPosition Responsibilities:
Provide low-barrier, harm-reduction-oriented medical care across TNM outreach settings, including Street Medicine, Health Outreach Bus, CTA outreach, shelters, and youth programs Conduct focused assessments and treat acute and chronic medical conditions in mobile and outdoor environments Initiate and manage pharmacologic treatment, including medications for opioid use disorder and other substance use treatments Provide wound care, vaccinations, preventive services, and health education, including participation in public health response efforts when needed Facilitate referrals and warm handoffs to primary care, specialty care, mental health, substance use treatment, and social services Maintain accurate and timely documentation in the Electronic Health Record and required reporting systems Collaborate with interdisciplinary outreach staff to coordinate client care and engagement Support medical supply management and medication inventory for outreach platforms Provide clinical guidance to medical students, residents, and volunteers Participate in quality improvement efforts, training, and required agency meetings Travel to various outreach locations and work in outdoor environments as required Perform additional duties as assigned This job description is intended to provide a general overview of the responsibilities and qualifications required for this position. Strong commitment to harm reduction and trauma-informed care Demonstrated experience working with individuals with substance use disorders Comfort practicing medicine in nontraditional, resource-limited environments Ability to make sound clinical decisions independently Excellent interpersonal and de-escalation skills Ability to exercise sound judgment and respond calmly in dynamic and emergency situations Strong organizational skills and attention to documentation detail Familiarity with community referral networks and safety-net systems Cultural humility and commitment to health equity Collaborative team mindset with flexibility to adapt to changing schedules, environments, and interdisciplinary outreach needsPreferred, Non-Essential Skills:
