Job Description

Nurse Practitioner - In‑Home Assessment (HouseCalls Program) We are seeking experienced Nurse Practitioners to perform in‑home health assessments for Medicare Advantage members. This role is independent, field‑based, and requires strong clinical judgment, excellent communication skills, and the ability to work in a wide variety of home environments. Current hot states include
AZ, AR, CA, CO, DC, ID, IL, IN, MO, NJ, NM, NY, NC, OH, OK, OR, TN, VA, WA.
Key Requirements Active NP license (unencumbered) National Board Certification
REQUIRED
AANP or ANCC Minimum 6 months NP experience - No new grads BLS/CPR required (ACLS not accepted) No show is paid at 50% of the assessment rate 10+ hours per week. Make your own schedule Board Certifications NOT Accepted Mental Health Pediatrics Acceptable NP Specialties Adult Gerontology Primary Care NP Adult NP Family NP Gerontology NP Acute Care NP (only accepted in specific states - see below)
