Job Description

We're expanding our team that delivers patient-centered care to residents in skilled nursing facilities across Cahokia and Hillsboro, IL. If you're a compassionate Nurse Practitioner or Physician Assistant who enjoys working with older adults in a collaborative, team-based setting, we'd love to hear from you.
Locations:
Hillsboro and Cahokia, IL, with additional openings nationwide in Massachusetts, Michigan, New Hampshire, North Carolina, Ohio, and Wisconsin.
What the role looks like:
Full-time positions with flexible schedules to help you balance work and life. Typical shifts are six hours inside the facility, with rounds starting between 7:00 a.m. and 10:00 a.m. No overnight or on-call responsibilities. You'll perform daily rounds, manage acute and chronic conditions, prescribe medications, and develop treatment plans. Documentation is streamlined with an EHR that includes an AI scribe and voice dictation; we provide an iPad with LTE. You'll also have access to UpToDate® for clinical decision support.
Compensation and benefits:
Competitive pay with an uncapped productivity-based model—strong potential to exceed industry benchmarks. Up to $50,000 in long-term retention incentives and a performance bonus program. Company-provided iPad with LTE and a lab coat. Eligible candidates may receive a sign-on bonus and relocation assistance.
Requirements:
Active, unrestricted Nurse Practitioner or Physician Assistant license. Current board certification (ANCC, AANPCB, PNCB, AACN, or NCCPA). Experience in skilled nursing, long-term care, geriatrics, or post-acute care is preferred.
