The ideal candidate will:

Manage a caseload of palliative patients, providing diagnosis, treatment, medication management, assessments, and consultations. Travel to patients' homes or care facilities to deliver personalized, dignified care. Develop and enhance our palliative care program, maintaining comprehensive knowledge in hospice and palliative practices. Requirements include a valid APRN license, RN license, NPI number or ability to obtain one, and willingness to collaborate with the Medical Director. Preferred experience is 1-2 years in nursing.
