Job Description
JOB OVERVIEW
We are seeking a dedicated and compassionate FNP to provide comprehensive healthcare to patients of all ages, focusing on preventive care, chronic disease management, and health education. This position begins as part-time with the intention to grow into a full-time role. Full practice authority holders are preferred.SCHEDULE & HOURS
TuesdayFriday:
9:00 AM- 5:00 PM (paid lunch 12:00
- 1:00 PM included)
Saturday:
9:00 AM- 1:00 PM 5 days per week, Tuesday
- Saturday; required from the start, including during part-time phase Position starts with morning hours (9:00 AM); schedule structure will be discussed and finalized during the hiring process as the role grows toward full-time
RESPONSIBILITIES
Conduct thorough patient assessments, including medical history and physical examinations. Develop and implement individualized care plans based on patient needs. Provide outpatient care, including diagnosis and treatment of acute and chronic illnesses. Administer medications and perform procedures in accordance with established protocols. Collaborate with physicians and healthcare professionals to ensure optimal patient outcomes. Educate patients and families on health management, disease prevention, and wellness strategies. Maintain accurate patient records using Athenahealth and Elation Health EHR systems. Participate in quality improvement initiatives and clinical guideline development.REQUIREMENTS
Master's degree inNursing :
Family Nurse Practitioner program. Current state licensure as a Family Nurse Practitioner. Basic Life Support (BLS) certification required. Experience in family medicine, geriatrics, or developmental disabilities is highly desirable. Familiarity with outpatient settings and teaching hospitals is an advantage. Strong interpersonal and communication skills with diverse patient populations. Commitment to continuous professional development and best practices in nursing care.BENEFITS 401
