This energetic and rewarding position offers the opportunity to collaborate with multidisciplinary teams, utilize advanced clinical skills, and positively impact patient outcomes. You will be at the forefront of acute and chronic disease management, providing expert assessments, treatments, and patient education in a fast-paced hospital environment. Your expertise will help ensure seamless care transitions, optimize patient recovery, and uphold the highest standards of medical excellence. Responsibilities Conduct thorough physical examinations and patient assessments to determine diagnosis and treatment plans. Manage acute medical care cases in General medical floors, transitional care unit and Acute rehab floor. Collaborate with physicians, nurses, therapists, and case managers to coordinate comprehensive patient care and discharge planning and attend daily Multidisciplinary rounds. Utilize Epic to document medical records accurately and efficiently. Ensure compliance with HIPAA regulations and while maintaining high-quality patient care standards. Experience Proven experience working in hospital medicine preferred. Demonstrated proficiency with EMR/EHR systems such as Epic. Strong background in managing Hospital medicine patients.
Pay:
$116,176.37 - $139,911.31 per year
