Manage a caseload of palliative patients, providing diagnosis, treatment, medication management, assessments, and consultations. Travel to patients' homes and facilities to deliver personalized, dignified care. Develop and expand our palliative care program while maintaining expertise in hospice and palliative practices. Requirements include a valid APRN license, RN license, and the ability to obtain an NPI number.
