Responsibilities:
Assess and monitor patients, diagnose and treat infectious diseases Prescribe medications, develop care plans, and coordinate care with physicians Ensure accurate documentation and address care gaps to improve outcomes Maintain a minimum of 12 patient visits daily (bonus for exceeding)
Qualifications:
Active Illinois NP license with a Master's degree in Nursing Preferred experience in infectious disease, geriatrics, or long-term care Strong communication skills, especially with elderly populations
