Job Description

WE ARE LOOKING FOR PROVIDERS TO ROUND WEEKLY OR BIWEEKLY IN SENIOR LIVING OCMMUNITIES IN THE FOLLOWING INDIANA LOCATIONS OF CARMEL, GREENWOOD, KOKOMO, AND CRAWFORDSVILLE.
We are seeking a dedicated and compassionate Nurse Practitioner to provide primary care services to our residents in assisted living, memory care, and independent living communities. The ideal candidate will be experienced in geriatric care, have a flexible schedule, and be comfortable working in a pay-by-production and monthly incentives along with yearly bonus structure. Hourly rate between $120-$240 per hour. This position can grow to full-time over time.
Responsibilities:
1. Provide comprehensive primary care services to a diverse patient population across various levels of care including assisted living, memory care, independent living, and skilled nursing facilities. 2. Conduct thorough health assessments and formulate treatment plans for acute and chronic diseases. 3. Collaborate with a multidisciplinary team of healthcare professionals to provide optimal patient care. 4. Prescribe and administer medication, therapy, and other specialized medical care to treat or prevent illness, disease, or injury. 5. Monitor and manage patient care plans, adjusting as necessary. 6. Educate patients and their families on health maintenance, disease prevention, and the management of acute and chronic illnesses. 7. Document patient care in compliance with regulatory standards and organizational policies. 8. Participate in quality improvement initiatives and contribute to the development and implementation of new health programs and services. 9. Provide a monthly updated Patient Census along with maintaining a weekly patient list
Qualifications:
1. Master's degree in Nursing from an accredited institution. 2. Valid Nurse Practitioner license and Registered Nurse license. 3. Certification in Geriatric or Adult-Gerontology Primary Care is preferred. 4. Proven experience providing primary care in a long-term care setting. 5. Strong clinical judgment, diagnostic skills, and excellent patient management abilities. 6. Proficient in electronic medical record systems. 7. Excellent communication and interpersonal skills. 8. Ability to work independently and as part of a multidisciplinary team. 9. Flexibility to adapt to changing schedules and patient needs.
Compensation:
Compensation will be based on a flexible structure, with options for pay-by-production and monthly incentives along with yearly bonus structure. Hourly rate between $120-$240 per hour.
