We are seeking a compassionate, motivated, and clinically strong Nurse Practitioner to join our growing Long-Term Care and Post-Acute Care team. This is an excellent opportunity for a provider who enjoys autonomy, flexibility, and building meaningful relationships with patients and facility staff, while still having close physician collaboration and support when needed. This role offers a flexible schedule, a high degree of independence, and the ability to make a genuine impact in the lives of geriatric and post-acute patients. Responsibilities Conduct comprehensive patient evaluations and follow-up visits in long-term care and post-acute care settings Assess, diagnose, and manage acute and chronic medical conditions Develop and implement individualized treatment plans Collaborate with physicians, nursing staff, therapists, and interdisciplinary care teams Provide timely and accurate medical documentation using EMR/EHR systems Participate in care coordination and communicate effectively with families and facility staff Assist in reducing hospital readmissions through proactive patient management Ensure compliance with HIPAA regulations and clinical best practices Qualifications Active Nurse Practitioner license in the state of practice Certification as an Adult-Gerontology, Family, or Adult Nurse Practitioner Long-term care, skilled nursing, hospital medicine, geriatrics, or post-acute care experience preferred Motivated new graduates are encouraged to apply Experience with EMR systems such as PCC, MatrixCare, GEHRIMED, or eClinicalWorks preferred Knowledge of ICD-10 coding and medical documentation standards Strong clinical assessment, communication, and organizational skills Ability to work independently while maintaining collaborative physician relationships Position Details 1099 Independent Contractor Position Flexible Scheduling Competitive Compensation Physician Support and Mentorship Available Opportunities for Professional Growth and Long-Term Collaboration Supportive Team Environment Minimal Administrative Burden Why Join Us?
