Job Description
Family Nurse Practitioner (FNP) Elira Health, LLC — Jasper, Indiana Job Summary Elira Health is seeking a compassionate and dependable Family Nurse Practitioner (FNP) to join our growing outpatient practice in Jasper, Indiana. The FNP will provide comprehensive primary-care services across the lifespan in a supportive, patient-centered environment. This is a contract position well suited for a provider who values clinical independence, collaborative care, and the opportunity to help shape a developing practice. Both full-time and part-time availability may be considered. Responsibilities Evaluate, diagnose, and treat acute and chronic health conditions Perform comprehensive and problem-focused examinations Provide preventive care, wellness visits, health screenings, and patient education Order and interpret laboratory tests, diagnostic studies, and imaging Prescribe and manage medications within the provider's scope of practice Develop individualized treatment plans and coordinate specialty referrals Perform appropriate in-office procedures based on training and competency Maintain accurate, timely, and compliant medical documentation Review test results and communicate follow-up recommendations to patients Collaborate with behavioral-health providers and other members of the care team Follow applicable clinical, prescribing, privacy, and quality standards Qualifications Master's or doctoral degree from an accredited nurse practitioner program Current national certification as a Family Nurse Practitioner Current Indiana RN and APRN licenses Current Indiana prescriptive authority DEA registration or eligibility to obtain one Current BLS certification Strong clinical judgment, communication, and organizational skills Ability to work both independently and collaboratively Commitment to respectful, evidence-based, patient-centered care Previous outpatient primary-care experience is preferred, but qualified new graduates may be considered.
