Job Description

Overview Join our dynamic healthcare team as an Advanced Practice Clinician, where your expertise will directly impact patient outcomes across diverse clinical settings. This role offers an exciting opportunity to utilize your comprehensive medical knowledge, critical care experience, and proficiency with electronic health record (EHR) systems to deliver high-quality, compassionate care. You will work collaboratively with physicians, nurses, and other healthcare professionals to assess, diagnose, and manage patient conditions efficiently and effectively. Your dedication to clinical excellence and patient-centered care will be vital in advancing our mission to improve health outcomes for all populations. Duties Conduct thorough patient assessments, including physical examinations, vital signs, and detailed medical histories using advanced clinical evaluation techniques. Manage acute and chronic conditions across various specialties such as internal medicine and rheumatology Perform diagnostic evaluations utilizing medical imaging, laboratory data collection, and interpretation of diagnostic tests Develop and implement treatment plans incorporating medication administration Utilize EMR systems like ECW for accurate documentation of patient records, coding (ICD-10/ICD-9), CPT procedures, and billing processes. Provide telehealth consultations to extend care access remotely while maintaining HIPAA compliance and data security standards. Refer patients to physicians as needed Maintain adequate records; periodically reviewing patient records in collaboration with the Medical Director. Knowledge, Skills Extensive knowledge of nursing principles and practices. Knowledge of pharmaceuticals to treat disease and illnesses, to include a knowledge of the mechanism of action, side effects, toxic nature, and drug interactions of medications. Ability to perform and interpret diagnostic tests in order to determine therapeutic treatment plans. Ability to exercise judgment in determining when to consult other resources for diagnosis and case management. Demonstrates accountability for nursing practice through planned & continuous professional growth and development. Demonstrates a leadership role through clinical practice, supervision, teaching and research. Ability to communicate effectively with patients, families, physicians, co-workers, and other personnel on a one-to-one basis The use of independent judgment and discretion while maintaining patient rights. The ability to multi-task and appropriately prioritize. Minimum Qualifications/Requirements Current Advanced Practice (APRN, NP or PA) license in the State of Kentucky Successful completion of a Nurse Practitioner or Physician Assistant program. AHA BLS for Healthcare Provider required.
