InstaMobile Care / Togo Health Job Overview We are seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to provide high-quality primary care services to patients in skilled nursing facilities (SNFs), assisted living facilities (ALFs), and private homes in Kentucky. This is a field-based role focused on delivering comprehensive, patient-centered care in post-acute and home settings. The ideal candidate is self-motivated, comfortable working independently, and passionate about improving healthcare access for vulnerable populations.
Key Responsibilities:
Conduct in-person medical visits at assigned nursing homes, assisted living facilities, and home settings Perform routine and acute care evaluations, chronic disease management, preventive screenings, and medication management Collaborate with facility staff, mobile medical assistants (MAs), and telehealth support to ensure coordinated, efficient care Document clinical encounters thoroughly and timely in the EHR system Order and interpret laboratory tests, diagnostics, and develop comprehensive care plans Communicate effectively with family members, caregivers, and interdisciplinary teams Adhere to evidence-based guidelines, organizational protocols, and all state/federal regulations Participate in regular case reviews, team meetings, and quality improvement initiatives
Qualifications & Requirements:
Active Kentucky provider license (NP or PA) DEA license Board certification (preferred) Current
ACLS/BLS
certification Valid driver's license and reliable transportation CAQH number Negative TB test (or proof of clearance) Strong clinical judgment and excellent communication skills Experience in geriatrics, post-acute care, or SNF settings is highly preferred Compensation & Schedule Competitive RVU-based compensation (daytime rate $12/RVU, nighttime rate $14/RVU) Flexible schedule options available Opportunity to join a rapidly growing multi-state provider group
