Job Description

Role & Responsibilities Authorized to prescribe and dispense medication when granted authority in accordance with State licensure Collect, order, and interpret lab work and diagnostic studies under agreed upon protocols May obtain comprehensive health histories, and perform complete physical examinations, develop differential diagnoses, develop and implement plans of care, evaluate and document client's response to plan of care May provide client and/or family education May organize referrals and/or consultations when appropriate Will promote cost effective delivery of care Will maintain active national certification through accredited certification body of choice Will maintain and be responsible for all requirements to keep APRN/PA license active yearly including patient hours and CEU's Outpatient Dialysis Responsibilities Manage the hemodialysis and/or peritoneal dialysis patient per Kidney Dialysis Outcome Quality Indicators (KDOQI) guidelines APRN/PA to work at least 40 hours per week to complete base of 60 patient touches per day Review number of visits periodically with goal for APRN/PA to perform 2-3 Basic visits per month and Comprehensive Visit when appropriate or designated by MD to do so Assess, diagnose, and manage systems-related complications of end-stage renal disease (ESRD) therapy including anemia, hypertension, bone disease, fluid and electrolyte imbalances, and vascular access Diagnose and manage episodic health problems of clients with ESRD Evaluate the client's progress and appropriateness of the plan of care Use an interdisciplinary team approach to plan comprehensive health care for the client Perform annual physicals including health maintenance and preventative health counseling Provide leadership to the clinical staff through education and consultation by serving as a role model and expert clinical resource in the nursing care of clients with ESRD Coordinate continuity of care between the outpatient dialysis unit and other healthcare settings Clinic Responsibilities May require travel to satellite office locations throughout Kentucky and/or Indiana Coordinate with Clinical Resource RN to help manage prescription refill per office protocol Manage EMR task box making patient calls regarding lab results, required future testing, and medication therapy as assigned daily by NP administrator Manage patients with chronic kidney disease, focusing on care that promotes health and prevents further decline in renal function Appropriate patients include established patient follow up, established patient hospital follow up, and renal replacement therapy education Coordinate with MD to provide quality patient care Perform scribe duties for MD in clinic as needed Provide educational materials that will allow the patients to become an active participant in his/her care and foster decision making abilities regarding treatments Hospital Rounds Coordinate(s) daily between NAK MD(s) covering hospital and establish plan for day and does not leave hospital until all assignments completed and/or excused by all MD(s) covering hospital that day Goal to see all consulted patient's in timely, efficient manner If appropriate and assigned by
MD, APRN
may start new consult APRN to remain in hospital during work hours Must be competent and efficient in use of Hospital EMR Assess the client and initiate encounter on EMR, coordinate referrals, and participate in discharge planning Educate and counsel clients on treatment options and expected outcomes Provide leadership to the clinical staff, serving as a role model and an expert clinical resource concerning nursing care of clients with chronic kidney disease, ESRD, and acute renal failure Coordinate continuity of care between the hospital and other healthcare settings Hours The APRN/PA will be available at least 40 hours per week to complete base of 60 patient touches per day. Staffing and operational demands may necessitate variations in beginning and ending times, as well as total hours scheduled per day and week. During regular work hours, the APRN/PA will be available for call from the office/hospital/clinics between the hours of 0800-1700. The APRN/PA will not be responsible for call on weekends/holidays. Qualifications and Education Requirements The Nephrology Hospital Advanced Practice Registered Nurse is masters or doctorate prepared as a Nurse Practitioner (NP) The Nurse Practitioner must have possession of a valid KY and/or IN license as a Registered Nurse and an Advanced Practice Registered Nurse, and be certified as a (Adult/Geriatric, Family OR Acute Care) Nurse Practitioner by an approved certifying body (ANCC or AANP). Certification as a Nephrology Nurse (CNN) or Certified Nephrology Nurse Practitioner (CNN-NP) is preferred, but not required. BLS Certified and ACLS Professionalism Professional behavior is expected to be demonstrated through the words, actions, and attitude towards fellow team members, patients, clients, and their families and all other members of the healthcare team. You are representing NAK at all times during business hours. Patient confidentiality will be maintained at all times. You will attend
NAK NP/PA
meetings on regular basis and will be accountable for checking all NAK correspondence in timely manner. Professional Growth As part of the medical health care team the APRN/PA will attend meetings as requested by NP administrator on a regular basis. A non-accumulative allowance specified by NAK per year per APRN/PA is allowed. Continuing education time and allowance necessary for re-certification and re-credentialing only. Involvement in a relevant professional organization is encouraged. Medical directors and NP administrator may make recommendations for continuing education toopics in the APRN/PA's evaluation. APRN/PA will at least meet minimum requirements by state(s) and certifying body per year. It is the responsibility of the APRN/PA to maintain these requirements and make sure CEU's are on file.
