On the road About Us Care First Mobile Medical Group Inc. is a leading mobile medical practice that provides high-quality primary care services to patients in the comfort of their homes throughout California. We primarily serve geriatric, homebound, and chronically ill Medicare patients. Our services include routine primary care, chronic disease management, post-hospitalization follow-up, oversight of home health plans of care, and coordination with home health agencies. Our Northern California Expansion Care First Mobile Medical Group is entering Phase 1 of its Northern California expansion. During this phase, we are recruiting qualified Nurse Practitioners in key regions to establish provider coverage. Phase 2 will focus on building referral relationships with home health agencies, assisted living communities, and other care partners throughout the region. As these relationships are established, providers will begin receiving patient assignments and developing patient panels within their service territories. This phased approach gives providers an opportunity to join our network early while we establish market partnerships and patient referral channels. Position Overview We are seeking experienced, compassionate, and motivated Nurse Practitioners to join our expanding mobile care team in Northern California. The ideal candidate has experience in home health, geriatric care, or family medicine and is passionate about delivering high-quality, patient-centered care in a home-based setting. This independent contractor position offers flexible scheduling, provider autonomy, and competitive compensation. Responsibilities Conduct comprehensive patient assessments, including medical histories, physical examinations, and screenings. Diagnose and manage acute and chronic conditions, including hypertension, diabetes, COPD, and heart disease. Develop and oversee patient-centered care plans in collaboration with patients, caregivers, and interdisciplinary care teams. Prescribe medications and order laboratory tests, imaging, and other diagnostic services when appropriate. Manage patients receiving home health services, including signing and overseeing home health plans of care. Educate patients and families about disease prevention, treatment plans, and health maintenance. Perform minor procedures, such as wound care, suture removal, injections, and skin biopsies. Communicate and coordinate care with physicians, specialists, home health nurses, and other healthcare professionals. Maintain accurate and timely electronic medical records. Travel to patients' homes throughout an assigned Northern California service area. Qualifications Active California Nurse Practitioner license. Valid DEA registration. Current Basic Life Support certification. Active professional liability insurance or willingness to obtain coverage. At least one year of home health experience preferred but not required. Experience in geriatric care or family practice strongly preferred. Reliable transportation and a valid driver's license. Strong clinical assessment and diagnostic skills. Excellent interpersonal, communication, and organizational skills. Ability to work independently and manage a mobile patient panel. Proficiency in additional languages is preferred but not required. Spanish, Vietnamese, Chinese, Farsi, Korean, Tagalog, Arabic, Armenian, and other language skills are especially valuable. Why Join Care First Mobile Medical Group?
