Join our expanding Post-Acute Care team on Cape Cod, providing patient-centered care for individuals transitioning from acute hospitals to skilled nursing and rehabilitation facilities. This full-time, facility-based role focuses on managing chronic and acute conditions, supporting recovery, reducing readmissions, and optimizing outcomes through close collaboration with interdisciplinary teams.

Barnstable, MA (primary); additional openings in Wakefield, RI and North Conway, NH. Schedule & Work Environment Full-time, Monday-Friday, 9:00 AM-5:00 PM No weekends or on-call responsibilities Facility-based rounding in post-acute care settings Clinical autonomy with strong operational support Opportunity to develop long-term relationships with patients and care teams Compensation & Benefits Base salary: $120,000 annually Total earning potential: $180,000-$220,000+ Comprehensive health, dental, and vision insurance Retirement savings plan Four weeks paid vacation Continuing Medical Education (CME) support Qualifications Graduate of an accredited NP or PA program Current, unrestricted NP or PA license in the applicable state, or eligibility for licensure National board certification (NP or PA) DEA registration or eligibility to obtain Minimum one year of clinical experience preferred; experience in post-acute care, skilled nursing, geriatrics, internal medicine, family medicine, hospital medicine, or long-term care strongly preferred Strong clinical assessment, diagnostic, and care coordination skills Excellent communication and interpersonal abilities Ability to work independently while collaborating effectively within multidisciplinary teams Proficiency with electronic medical record (EMR) systems If you are a compassionate, motivated NP or PA committed to high-quality post-acute care and value autonomy with collaborative support, we encourage you to apply.
