Job Description
Community Health Programs Inc, a nonprofit serving the Berkshires, seeks a Primary Care Provider - Physician to deliver comprehensive, patient-centered care to diverse and underserved populations. In this team-based role, you will provide acute, chronic, and preventive primary care, manage chronic diseases, and coordinate with behavioral health, dental, and specialty services. You will use EHRs, contribute to quality and equity initiatives, and collaborate closely with an inclusive, mission-driven care team dedicated to accessible, high-quality community health. Responsibilities Provide comprehensive primary care to pediatric through adult patients, including acute, chronic, and preventive services. Conduct histories, physical exams, diagnosis, and evidence-based treatment planning. Manage chronic diseases and coordinate care with behavioral health, dental, and specialty services. Document all encounters thoroughly in the EHR and comply with quality and safety standards. Participate in team-based care, case conferences, and population health initiatives. Provide patient education, preventive counseling, and health promotion. Contribute to quality improvement, equity, and access initiatives aligned with CHP's mission. Participate in shared call coverage and occasional extended hours as needed.
