Job Description The Nurse Practitioner (NP) or Physician Assistant (PA) will provide comprehensive primary care services with a specialized focus on evidence‑based hormone replacement therapy for both men and women. This role blends traditional preventive medicine with modern longevity and metabolic optimization practices. The NP or PA will evaluate, diagnose, and manage acute and chronic conditions, develop individualized hormone protocols, monitor treatment response, and collaborate with the clinical team to deliver high‑quality patient care. Core Responsibilities Primary Care Perform comprehensive history, physical exams, and annual wellness visits Diagnose and manage common acute and chronic conditions (e.g., hypertension, diabetes, thyroid disorders, dyslipidemia) Order, interpret, and act on laboratory tests, imaging, and diagnostics Develop evidence‑based treatment plans and follow‑up schedules Provide patient education on lifestyle, nutrition, sleep, stress, and preventive care Coordinate referrals to specialists when clinically appropriate Maintain accurate, timely documentation in the EMR Hormone Replacement Therapy (Men & Women) Conduct detailed hormone evaluations including symptom review, medical history, and risk assessment Order and interpret hormone panels (e.g., testosterone, estradiol, progesterone, DHEA, SHBG, thyroid markers, cortisol) Develop individualized HRT plans using modalities such as: Testosterone therapy for both males and females Estrogen and progesterone therapy Adjunctive therapies (thyroid support, adrenal support, DHEA, peptides, lifestyle interventions) Monitor treatment response, adjust dosing, and track safety markers Educate patients on risks, benefits, expectations, and long‑term management Identify and manage side effects or complications Collaborate with practice owner on complex cases or protocol updates Patient Experience & Communication Build strong therapeutic relationships with patients seeking long‑term optimization Communicate clearly and compassionately about treatment plans and expectations Provide ongoing support through follow‑ups, messaging, and care coordination Maintain a patient‑centered, empathetic, and professional approach Collaborate with administrative staff to ensure smooth clinic flow Qualifications (Required) Active MA license, national board certification, DEA license, and MCSR Experience in primary care Prior experience in hormone replacement therapy for both men and women

$55.00 - $75.00 per hour
