Position:
This is full performance specialized professional community health nursing work providing preventive or management health care services in an expanded nurse role as a nurse practitioner to individuals, families, and groups. In addition, work is performed to address the health care needs of target populations at risk or have chronic disease such as obesity or diabetes. The incumbent provides health teaching, behavioral counseling and guidance to patients and their families and conduct behavioral health assessments and evaluations. Work is done in accordance with the Maryland Nurse Practice Act Code of Maryland Regulations Practice of the Nurse Practitioner. About the Agency The Prince George's County Health Department's (PGCHD) vision is that all Prince Georgians are their healthiest at every age and every stage. Their mission is to lead, engage, and empower our community to work collaboratively towards disease prevention, health equity, and total well-being. The PGCHD works to protect and support the public's health through numerous services that range from restaurant inspections and disease tracking to care coordination and health promotion. The department is composed of four divisions: Behavioral Health Services, Environmental Health and Disease Control, Family Health Services, and Health and Wellness, supported by the Office of Administration, Office of Human Resources and the Office of the Health Officer. The mandate of the department has been, and will remain, broad. This plan emphasizes improving core functions and operations, including protecting the public from health threats, implementing strategies to support healthy living, and maintaining a focus on vulnerable populations, while developing efficient internal processes to support these activities. Collaborate with pediatricians, dietitians, psychologists, mental health providers and other healthcare professionals to design and implement comprehensive evidence-based care plans for patients at risk for or managing chronic conditions such as diabetes, obesity, cardiovascular disease, and hypertension. Assist with recruiting practices, community-based organizations, pharmacies, and eligible participants to participate in various evidence-based chronic diseases programs. Develop and implement individualized educational plans for patients and their families, emphasizing healthy lifestyle choices, nutrition, and physical activity. Provide ongoing counseling and support to encourage positive behavior changes. Maintain accurate and timely documentation of therapy sessions, treatment plans, and patient progress. Assist Program Chief with administering evidence based chronic disease programs which focus on diabetes prevention and management and obesity. Implement the Family Healthy Weight Program known as Family-based Behavioral Treatment. Train practice staff on use of standardized protocols and guidelines to improve development of care plans that aim to promote effective management of chronic diseases. Perform health screenings according to established protocols, which may include measuring blood pressure, glucose levels, cholesterol levels, BMI, and other relevant measurements. Graduation from a National League for Nursing accredited college or university or an equivalent program as determined by the Maryland State Board of Nursing examiners with a Bachelor's Degree in nursing. Possession of a current license to practice as a Registered Nurse. Possession of a valid motor vehicle operator's license. AND/OR Master's Degree in public health, community health nursing or other appropriate nursing specialty. Successful completion of a Nurse Practitioner Program and certification as Nurse Practitioner in the State of Maryland issued by the Maryland State Board of Nursing. Possession of a valid motor vehicle operator's license. An equivalent combination of education and experience may be substituted for the master's degree, but not for the successful completion of a Nurse Practitioner Program. Preferred Qualifications Experience with Epic EMR patient records.
