Job Description
Primary Care Nurse Practitioner Job Requirements The Primary Care Advanced Practice Provider provides comprehensive, patient-centered primary healthcare services in collaboration with physicians and the interdisciplinary healthcare team. The APP evaluates, diagnoses, treats, and manages patients across the lifespan in the outpatient primary care setting, emphasizing preventive care, chronic disease management, health promotion, and continuity of care. The APP functions as an integral member of the ambulatory care team and supports the delivery of high-quality, evidence-based care through comprehensive assessment, medication management, preventive health services, patient education, and coordination of care. The APP manages both acute and chronic medical conditions while promoting wellness and disease prevention. The APP collaborates with physicians, specialists, nurses, care managers, behavioral health professionals, pharmacists, and community resources to ensure coordinated and effective patient care. The APP practices within the scope of professional licensure, national certification, institutional credentialing, and delineation of privileges in accordance with applicable state regulations and hospital policies. Work Experience Graduate of an accredited Nurse Practitioner or Physician Assistant program . Current and unrestricted state licensure as a Nurse Practitioner or Physician Assistant . Current national certification through an approved certifying body ( ANCC, AANP, or NCCPA ). Current Basic Life Support (BLS) certification required. Prior experience in primary care, family medicine, internal medicine, ambulatory care, or community health preferred. Experience managing chronic medical conditions and preventive care services preferred. Experience working with diverse patient populations preferred.
