Job Description

Location:
Charlevoix, MI (additional facilities in Carson City, Crystal Falls, East Lansing, Escanaba, Iron River, Mt. Pleasant, South Haven).

Role summary Provide patient-centered primary and acute care to residents in skilled nursing and post-acute facilities. Work in a collaborative, team-based environment focused on geriatrics and long-term care. Key responsibilities Perform daily rounds (typically start between 7:00-10:00 a.m.) and six-hour facility shifts for full-time providers Evaluate and manage acute and chronic conditions Prescribe medications and develop treatment plans Coordinate with nursing staff, physicians, and interdisciplinary teams Document care using company EHR (AI Scribe, voice dictation) on company-provided iPad with LTE Use UpToDate® for clinical decision support No overnight shifts or on-call duties Schedule Full-time opportunities with flexible scheduling Six-hour facility shifts; no call or overnight responsibilities Compensation & benefits Competitive base pay with uncapped productivity-based upside Up to $50,000 in long-term retention incentives Performance bonuses and possible sign-on bonus Paid Time Off 401(k) with employer match Medical, dental, vision, life, and disability insurance (eligible providers) CME allowance and licensure reimbursement Company-provided iPad with LTE and lab coat Relocation assistance available for eligible candidates Qualifications Active, unrestricted Nurse Practitioner license National certification (ANCC, AANPCB, PNCB, AACN, or equivalent) Skilled nursing, long-term care, geriatrics, or post-acute care experience preferred To apply Submit resume and current licensure/certification documentation.
