Provide education to patients and families regarding health management, disease prevention, and wellness strategies. Collaborate with interdisciplinary teams to coordinate patient care across various settings, including clinics and telehealth services. Monitor patient progress and adjust treatment plans as necessary, utilizing medical terminology effectively. Perform procedures such as tube feeding and sonography when required. Ensure compliance with infection control protocols and best practices within the clinical environment. Maintain accurate and up-to-date patient records using electronic health record systems like Cerner. Qualifications Master's degree in Nursing with a focus on Family Nurse Practitioner program. Current certification as a Family Nurse Practitioner (FNP) from an accredited board. Strong knowledge of geriatrics, memory care, and related healthcare practices. Familiarity with telehealth technologies and their application in patient care. Proficient understanding of medical terminology and infection control measures. Experience working in clinical settings is preferred; familiarity with Cerner or similar systems is a plus. Excellent communication skills, both verbal and written, with the ability to educate patients effectively.
