Your schedule is Monday through Friday, day shifts, with no nights or weekends required. As a key member of a multidisciplinary team, you play a central role in patient care. You'll spend your days managing patients with acute and chronic diseases, collaborating directly with the attending physician to provide the highest quality of care. Your expertise is crucial in effectively managing patients with debilitating chronic diseases, improving their quality of life. You are a natural collaborator, communicating and working seamlessly with the Director of Nursing, Social Workers, and other team members to ensure a cohesive and comprehensive approach to care. This dynamic role keeps you engaged and connected as you make a tangible difference in the lives of those you serve.OUR IDEAL PRACTITIONER A
board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP) 3-5 years of experience as an RN or post-acute role specializing in pulmonary 1+ years of post-acute, long-term, and/or skilled nursing facility experience in pulmonaryWHO WE ARE
