In this vital role, you will deliver high-quality, patient-centered care across a diverse range of medical settings.. Your expertise will help improve patient outcomes through comprehensive assessments, diagnostic evaluations, and tailored treatment plans. Duties Conduct thorough physical examinations and patient assessments to determine health status and diagnose medical conditions. Prescribe medications and treatment regimens in accordance with state regulations and collaborative practice agreements. Manage acute and chronic illnesses Utilize electronic health record (EHR) systems like Epic to document patient encounters accurately and efficiently. Collaborate closely with physicians, specialists, therapists, and other healthcare professionals to coordinate comprehensive patient care. Skills Extensive experience working with diverse patient populations including seniors, individuals with disabilities, and those in assisted living or hospice. Proficiency in EMR/EHR systems such as Epic. Strong knowledge of medical terminology, physiology, anatomy, CPT coding (Current Procedural Terminology), ICD coding (International Classification of Diseases), ICD-10 classifications for accurate diagnosis coding. Knowledge of HIPAA regulations for patient privacy; excellent client communication skills; ability to educate patients on health coaching and medication administration. Capable of managing complex cases involving workers' compensation law, discharge planning, utilization review, case management or post-acute care coordination.
