In this vital role, you will provide comprehensive patient care across diverse settings, utilizing your advanced clinical skills to assess, diagnose, and treat a wide range of health conditions. Your expertise will help improve patient outcomes, promote wellness, and support our mission to deliver exceptional healthcare services. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to inform diagnosis and treatment plans. Manage acute and chronic conditions through medication administration, injections, IV infusions, suturing, and other advanced procedures such as dermal fillers and spinal taps. Collaborate with physicians, specialists, and multidisciplinary teams to develop personalized care strategies for patients across outpatient, inpatient, and long-term care settings. Utilize electronic health record (EHR) systems such as Epic, Cerner, or eClinicalWorks for accurate medical documentation, coding (ICD-10, CPT), and case management. Provide patient education on health coaching topics including nutrition, medication adherence, disease prevention, and post-discharge care planning. Participate in clinical research initiatives and quality improvement projects to enhance treatment protocols and ensure compliance with healthcare standards like NCQA or HIPAA regulations. Requirements Valid Nurse Practitioner licensure with current certification to practice in your state; additional certifications in acute care or family practice preferred. Proven experience working in diverse healthcare environments such as hospitals (Level I Trauma Center or Level II Trauma Center experience), nursing homes, assisted living facilities, or outpatient clinics. Strong proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth; familiarity with medical coding (ICD-9/10) and CPT coding is highly desirable. Demonstrated skills in performing complex procedures such as suturing, catheterization, spinal taps, dermal fillers, and sterile processing techniques. Critical care experience including ICU or PICU settings; experience with dialysis, ventilator management, or neonatal ventilators is advantageous. Knowledge of pharmacology related to psychopharmacology, pain management, infectious disease care, hospice & palliative medicine; familiarity with Workers' Compensation law is a plus. Excellent communication skills for patient assessment and education; ability to work effectively within multidisciplinary teams while maintaining compliance with HIPAA standards.
