Job Description

Location:
South Haven, Michigan (additional facility opportunities in Carson City, Charlevoix, Crystal Falls, East Lansing, Escanaba, Iron River, Mt. Pleasant). Additional statewide opportunities in IL, NH, NC, OH, and WI.
Overview:
Provide high-quality, patient-centered care to residents in skilled nursing and post-acute settings. Work as part of a collaborative team to manage acute and chronic conditions in older adults.
Schedule and clinical duties:
Full-time positions with flexible scheduling. Six-hour facility shifts (rounds typically start between 7:00-10:00 a.m.). No call or overnight responsibilities. Daily rounds, assessment and management of acute and chronic conditions, medication and treatment plan prescribing. Efficient documentation using streamlined EHR with AI scribe and voice dictation.
Technology and clinical support:
Company-provided iPad with LTE and lab coat. Access to UpToDate® for clinical guidance.
Compensation and benefits:
Competitive base pay with uncapped productivity-based earnings. Up to $50,000 in long-term retention incentives. Performance bonuses and sign-on/relocation assistance for eligible candidates. Paid Time Off; 401(k) with employer match. Medical, dental, vision, life, and disability insurance (eligible providers). CME allowance and licensure reimbursement.
Requirements:
Active, unrestricted Nurse Practitioner or Physician Assistant license. Board certification (ANCC, AANPCB, PNCB, AACN, or NCCPA). Skilled nursing, long-term care, geriatrics, or post-acute care experience preferred.
