$100,000 - $140,000 per year Position Summary HomeMD (transitioning into HomeNP) is seeking compassionate and clinically skilled Nurse Practitioners to provide primary care services to seniors and medically complex patients in residential settings throughout Central Oakland County. Our providers deliver healthcare directly to patients in their homes, assisted living communities, and other care environments, helping improve access to care while reducing unnecessary hospital and emergency room visits. Providers typically work within a localized territory near their residence, with most travel occurring within approximately 45 miles of their home base. Limited travel beyond this range may occasionally be required. Areas Served Troy, Rochester Hills, Auburn Hills, Bloomfield Hills, Birmingham, Royal Oak, Clawson, Madison Heights, Southfield, Pontiac, Waterford, West Bloomfield, Farmington Hills, Lake Orion, and nearby communities. Key Responsibilities In-Home Primary Care Perform comprehensive health assessments and follow-up visits Evaluate acute and chronic medical concerns Develop, implement, and update individualized treatment plans Order and interpret laboratory and diagnostic testing Chronic Disease Management Monitor and manage conditions such as diabetes, hypertension, COPD, heart failure, arthritis, and dementia Track patient progress and adjust treatment plans as needed Promote preventive interventions and disease management strategies Medication Oversight Review medications for effectiveness, safety, and compliance Coordinate medication adjustments with pharmacies and collaborating providers Educate patients and caregivers regarding treatment plans and medication use Preventive Health Services Conduct annual wellness visits and routine screenings Identify health risks and implement preventive care measures Promote healthy aging and patient independence Care Coordination Collaborate with physicians, specialists, caregivers, facility staff, and support services Ensure continuity of care following hospitalizations or significant health events Maintain accurate and timely clinical documentation Patient & Family Education Provide guidance regarding chronic disease management, nutrition, mobility, safety, and overall wellness Support caregivers and family members in understanding care plans and health goals Services Frequently Provided In-home primary care visits Chronic care management Annual wellness visits Transitional care following hospital discharge Behavioral health monitoring and support Fall risk assessments Basic wound and skin evaluations Diagnostic testing and laboratory ordering Clinical Support HomeMD providers are supported by an experienced team of Medical Assistants, Care Coordinators, and administrative staff who assist with scheduling, patient outreach, documentation support, and care coordination activities. Qualifications Active Michigan Nurse Practitioner license Board certification as a Nurse Practitioner Master's or Doctoral degree in Nursing Strong assessment, clinical decision-making, and communication skills Experience in geriatrics, primary care, home-based care, palliative care, internal medicine, or related specialties preferred Preferred Background Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP), Geriatric Nurse Practitioner, Advanced Practice Provider (APP), or similar advanced practice experience.
