36 Hours per Week | Rotating 12-Hour Shifts Position Overview We are seeking an experienced Advanced Practice Provider (APP) - Nurse Practitioner (NP) or Physician Assistant (PA) to join an established onsite employee health center in Austin, Minnesota. This opportunity is available as either a contract or contract-to-permanent position . The health center is currently recruiting for a permanent replacement for a long-standing APP who is retiring, making this an excellent opportunity for a provider interested in potentially transitioning into a permanent role. The Health Center provides accessible, comprehensive primary care services to employees and their dependents, ages 2 and older , supporting both production and corporate employees and their families. Schedule 36 hours per week - rotating 12-hour shifts bi-weekly Week 1: Monday, Tuesday, Wednesday Week 2: Wednesday, Thursday, Friday Key Responsibilities The APP will provide comprehensive primary and preventive care to patients ages 2 and older , including: Conduct annual physicals and routine preventive health examinations Provide acute and same-day care for illnesses and minor injuries Perform well-woman examinations Conduct well-child visits and sports physicals Administer and manage immunizations Diagnose and manage common acute and chronic health conditions Perform appropriate minor procedures Order, perform, and interpret laboratory and point-of-care testing (POCT) Provide medication management and provider dispensing as appropriate Develop individualized treatment and preventive care plans Provide patient education related to health, wellness, disease prevention, and chronic condition management Coordinate care with internal clinical resources and outside providers when necessary Accurately document patient encounters within the electronic medical record Health Center Services The onsite Health Center offers a comprehensive range of services, including: Primary and preventive care Annual physical examinations Well-woman examinations Well-child visits Sports physicals Immunizations Acute and same-day visits Minor procedures Laboratory services Point-of-care testing (POCT) Provider dispensing Physical Therapy Care Management Health and wellness education Collaborative Clinical Environment The APP will be supported by an established multidisciplinary team that includes: Reception/front-office team LPN and Medical Assistant clinical support staff RN Health Center Manager RN who partners closely with the Primary Care APPs to provide patients with ongoing health and wellness education and support Qualifications Licensed Nurse Practitioner (NP) or Physician Assistant (PA) Appropriate active Minnesota licensure and credentials required to practice Experience providing care across a broad patient population, including pediatric and adult patients Primary Care experience strongly preferred Epic EMR experience preferred Strong clinical assessment, diagnostic, and patient education skills Ability to independently manage routine primary care and acute-care visits Excellent communication and interpersonal skills Collaborative, patient-focused approach to care Ideal Candidate The ideal provider is comfortable caring for patients across the lifespan, enjoys practicing in a collaborative primary care environment, and is passionate about preventive health and building relationships with patients.
